Federal-State Interoperability – The DMES Bidirectional Interface

Upon joining Therap Services LLC in 2021, I inherited a critical operational gap for the Delaware Division of Developmental Disabilities Services (DE DDDS). While the state utilized our platform as their primary EHR, they lacked real-time access to essential Medicaid eligibility, Managed Care Organization (MCO) assignments, and funding source data—information housed exclusively within the Delaware Medicaid Enterprise System (DMES). This data fragmentation forced state staff to manually verify eligibility, creating a significant bottleneck in day-to-day business operations and delaying mission-critical service authorizations.

To resolve this, I was tasked with designing and leading the implementation of a bidirectional, SFTP-based interface to act as a bridge between the two systems. I functioned as the primary Business Analyst and QA Lead, responsible for translating the state’s highly complex business logic into a functional system design. The architecture required the generation of fixed-length request files following specific state-mandated sequences, which were then transmitted via secure SFTP for DMES processing. I engineered the return logic to pull DMES response files in a timed fashion, ensuring that once processed, the data was stored in a reportable and user-friendly format within the EHR.

Throughout the Release Management cycle, I placed a heavy emphasis on Risk Management and transparency. I designed a sophisticated error-handling framework that provided the state with granular visibility into any exchange failures, allowing them to make informed, cognitive decisions based on the data received. To ensure a seamless transition, I staged a comprehensive UAT involving both DE DDDS and DMES technical units. This multi-party validation ensured that every stakeholder was aligned on the technical specifications and functional outcomes before the interface was moved to production.

The implementation of the DMES interface successfully eliminated manual data entry for the state, providing an automated "Source of Truth" for Medicaid eligibility. By establishing this high-security, industry-standard exchange, I empowered DE DDDS leadership to make critical business decisions based on real-time data. The project stands as a benchmark for successful state-to-vendor interoperability, characterized by a 100% success rate in data synchronization and a significant increase in operational efficiency for Delaware’s disability services.
